The Roosevelt Island shootout occurred in the spring of 2008 when a three-way shootout between the NYPD/SWAT, the Spanish Lords/Angels of Death, and lone gunman Niko Bellic erupted at the old hospital on Roosevelt Island, New York City. Bellic was sent by Elizabeta Torres to retrieve some Bolivian cocaine that had been stolen from her by Spanish Lords drug dealer Oscar Gaos, who had been introduced to her by Little Jacob. Gaos met with AOD biker Johnny Shortman and attempted to sell the drugs to him, but Bellic assaulted the old hospital, killing several bikers and Puerto Rican gangsters. After all of the gangsters were dead, Bellic stole the cocaine suitcase, but the SWAT and NYPD launched a special operation to retrieve the cocaine and apprehend Bellic after undercover CIA agent Karen Daniels managed to discover Torres' involvement with the drugs. Bellic killed several policemen and SWAT troopers and fled to South Bronx, where Little Jacob requested his presence; Jacob felt that something was awry. It was there that Daniels confronted Bellic and had him hand over the cocaine to her, and Bellic walked away empty-handed. The result of the shootout was over a dozen deaths, as well as the imprisonment of Elizabeta Torres for 300 years on cocaine distribution charges.
