Patrick "Packie" Richard McReary (June 29, 1979-) was the second-in-command of the Irish Mob in Queens, New York City under his brother, Gerald McReary. The McReary crime family was involved with the American Mafia during the 2000s, and McReary and his brothers worked with the Pegorino crime family to fight against the Ancelotti crime family, keeping the declining Irish Mob afloat.

Patrick McReary was born in Astoria, Queens, New York City in June 29, 1979 to an Irish Catholic family, the son of John and Maureen McReary. Patrick was the younger brother of Derrick McReary, Francis McReary, and Gerald McReary and the older brother of Katie McReary, and the brothers were frequently abused and molested by their father. In 1993, at the age of fourteen, McReary dropped out of school to roll marijuana joints for his father, and he became involved with crime at a young age. In 1997, he was arrested for grand theft auto at the age of 18, and he was suspected in a number of robberies and hijackings; he was arrested again in 1998 for grand larceny and in 2000 for hijacking. McReary was known to have a drug habit, and he was arrested in 2005 for possession several grams of cocaine as he spent time with a prostitute.

McReary worked as muscle for the New Jersey-based Pegorino crime family in addition to his own family, the McReary crime family, and he was also associated with South Bronx drug dealer Elizabeta Torres. In 2008, he befriended Niko Bellic while the two were working with Torres - Bellic saved McReary and his associate Peter O'Conrad from a shootout with Ignacio Maroto and the Spanish Lords - and Bellic helped McReary with a bank robbery and with his work for the Pegorinos; the two killed several top Ancelotti crime family leaders and attempted to revive the dying Irish Mob. The two also kidnapped Gracie Ancelotti and forced Tony Prince to meet them on Randalls and Wards Islands to hand over $2,000,000 worth of diamonds in exchange for the girl; the Bulgarin crime family ambushed the exchange, and McReary and Bellic fought their way out of the ambush, leaving empty-handed. McReary lost his brother Francis McReary to Niko Bellic's sniper bullet (unbeknownst to him), Derrick to drugs, and Katie to a drive-by shooting carried out by Jimmy Pegorino while Katie, Patrick, and Niko were attending Roman Bellic's wedding, and Gerald was imprisoned at the East Jersey State Prison; Patrick McReary decided to leave for California and start over. He was involved in some small robberies before being recruited into Michael De Santa's robbery crew, taking part in several major heists after De Santa rescued him from a botched convenience store heist.
