Kiyomi as a hostess
Kiyomi Kasahara was born in Onomichi, Hiroshima Prefecture, Japan in 1975, and she was high school classmates with Kazuaki Ino and Tsuyoshi Nagumo, who fought for her affection. After high school, Kiyomi moved to Tokyo and got a job as a hostess, becoming rich from the bubble economy. When she turned 30, she met a customer who tricked her into debt, but she was saved by Takumi Someya, who proceeded to start a relationship with her, marry her, and father a child, Hiromi. Kiyomi came to be terrified of Someya's violent yakuza lifestyle, and she suffered physical abuse from Someya whenever she attempted to run away. She was ultimately forced to abandon her child and flee back to Onomichi by herself, opening up Snack Kiyomi.
In 2013, Kiyomi gave the wandering Haruka Sawamura a job and a place to stay at Snack Kiyomi, taking pity on her loneliness and desperation. The two built a good friendship, but Haruka suddenly disappeared in 2015 after learning that she was pregnant. In December 2016, Kazuma Kiryu came to Onomichi to look for his adoptive daughter, and Kiyomi - while initially denying knowing Haruka - arranged for Ino to let Kiryu stay at the Navy Apartments as he searched for the infant Haruto Sawamura's father following Haruka's injury in a car accident. Nagumo repeatedly picked fights with Kiryu, jealous over Kiyomi's admiration of Kiryu, but Kiryu later saved him from the Masuzoe-gumi after he took a beating rather than collect protection money from Kiyomi, causing the two men to build a friendship of their own.

Kiyomi in 2016
Not long after, Someya came to town to swear an oath with Yomei Alliance captain Kanji Koshimizu and kidnapped Kiyomi to lure Kiryu to his hotel, where Yomei chairman Heizo Iwami wanted Kiryu to bear witness to Someya and Koshimizu's oath. Kiyomi was surprised to see Kiryu arrive, as she had not been told of her ex-husband's plan. Ultimately, Kiryu refused to witness the oath, and, when he set out for Kabukicho, Tokyo to track down Shu Tatsukawa - allegedly Haruto's father - he persuaded Kiyomi to take care of Haurto in his absence. When the trio returned, Kiyomi expressed her regret for leaving Hiromi with Someya, feeling she failed as a parent, but Nagumo assured her she did the right thing. She later attended Toru Hirose's funeral and was relieved to hear from Kiryu that Haruka had regained consciousness on being reunified with Haruto. However, she was taken hostage by Tsuneo Iwami not long after, with Iwami and Katsumi Sugai ordering Kanji Koshimizu to kill her if Someya failed to kill Kiryu in a fight at the Millennium Tower. Sure enough, Kiryu defeated Someya, who, desperate to keep Kiyomi alive, committed seppuku. However, Tsuneo still gave the order for Koshimizu to kill Kiyomi. Koshimizu, who refused to kill Kasahara, instead fired blanks at her to give Tsuneo the idea that she had been executed. Not long after, once Tsuneo and his conspirators were defeated, she was reunited with Hiromi, whom she raised with Nagumo's help.
